The scale is an elementary or primary lesion which consists of the
detachment or desquamation of the superficial layers of the epithelium
in a visible manner, while keratosis describes the accumulation of
scales which do not slough off. Included
in the “scalous” entities we have exfoliative cheilitis, chronic
lupus erythematosus, psoriasis and Gilbert’s pink pitiriasis.
Some conditions which produce scaling may also be included in the
keratotic lesions.
From a diagnostic and prognostic point of view, keratosis is described
as a white, engrossed mucosa. Conditions which lead to congenital
malformation may have keraatotic lesions, but it is more frequently seen
in grade II leukoplakia and keratotic (hypertrophic) lichen planus.
